Telehealth in the form of RPM has shown to be useful & associated with reduced hospital admissions, nursing home admissions, ED visits, and improved preventative care. Chronic care is moving from the clinic and into the home, so the intersection of telehealth and home health care is a keystone to successful adaptation.

Home health is also associated with reduced patient mortality and hospitalizations. The care continuity in health systems managing patients with chronic illness depends on continued prescription access & how this changes in a virtual setting.

Given telehealth's potential to affect chronic illness management, the authors share their telehealth policy & administration recommendations that follow TTP in this patient group.
